延髓背外侧综合征 14 例临床分析

【摘要】目的分析延髓背外侧综合征(Wallenberg′s syndrome)的临床特征、治疗及预后。方法回顾性分析 14
例临床诊断为延髓背外侧综合征患者的临床资料。结果本组均为急性起病,以眩晕、恶心、呕吐、延髓麻痹及感觉障碍
为主要临床表现。14 例行 MRI 检查,11 例显示延髓梗死。DSA、CTA 检查可明确病变部位及程度。本组 4 例行 DSA 检查,
1 例行 CTA 检查,其中 4 例显示不同程度椎动脉病变。14 例采用抗凝、抗血小板、降压、降糖治疗,14 例症状均缓解,7 例
遗留不同程度延髓麻痹、感觉障碍及霍纳综合征。结论 MRI 对诊断有较高敏感性,DSA、CTA 检查可明确病变部位及程
度。经治疗后大多数病例预后良好。

【Abstract】 Objective To analyze the clinical features,therapy and prognosis of Wallenberg′s syndrome. Methods The
clinical data in 14 cses of Wallenberg′s syndrome were retrospectively analyzed. Results The cases presented with acute onset.
The principal main manifestations were dizziness,nausea,vomiting,bulbar paralysis and sensory disturbance. In 14 cases of MRI
examination,11 cases demonstrated bulbar infraction. 4 cases and 1 case respectively received DSA and CTA examinations,and
