THE CLINICAL STUDY OF PERCUTANEOUS DISC DECOMPRESSION OF TREATING HERNIATION OF CERVICAL DISC WITH Nd:YAG LASER

Objective: To study the possibility of PLDD (percutaneous laser disc decompression) and to investigate an ideal non-operative method which has long lasting effect for PLDD.
Methods: 35 patients of cervical disc herniation with PLDD were studied. All the herniated discs were irradiated with 10~15J/S Nd:YAG laser quantum by photic-fiber under the supervision of C-type Xray.

Results: All the patients were followed and reexamined CT or MRI after one to six months of PLDD. The cured (%), excellent(%), moderate(%), non-effect(%). The excellent was 80%. The effective rate was %. Non-effective rate was %.

Conclusion: When irridiated with Nd:YAG laser, the neucleus pulposus was vapouring, charring and coagulating. The volume and innerpressure of the disc decreased. So the symptoms and signs improved. The main value of this methods were micro-damage, non-operation, no bleeding, no bone injury, good theraputic effect, quick recovery, lesser pain, safety and excellent long lasting effect. It is an ideal non-opertive method of treating PLDD.

为了研究经皮激光颈椎间盘减压术(Percutaneous Laser Disc Decompression . 以下简称PLDD术)的可行性， 探讨微创伤、痛苦小、恢复快、效果佳以及远期效果理想的非手术疗法治疗颈椎间盘突出症，自2000年6月至今，我们采用日本产Nd:YAG Laser CL-50激光器、在日立105C型臂X光机监控下共做经皮激光颈椎间盘减压(PLDD)术35例，取得了令人满意的临床治疗效果。现报道如下：

1 临床资料
一般资料 本组35例中，男性21例，女性14例；年龄35-76岁，；病程1个月-6年。17例有明确外伤史，26例有上肢麻木史（其中单侧上肢麻木15例；双侧上肢麻木11例）；C3-4椎间盘突出7例，C4-5椎间盘突出9例，C5-6椎间盘突出11例，C6-7椎间盘突出8例，两个椎间盘同时突出者10例，三个椎间盘同时突出者12例，共施行PLDD术62个椎间盘。

病例的选择 所选病例均经CT或MRI影像学检查，且临床症状、体征等符合颈椎间盘突出症的诊断标
