真菌性角膜炎角膜新生血管形成与预后的研究

摘要

目的通过观察兔眼角膜真菌感染后角膜新生血管形成规律,探讨角膜新生血管形成与
角膜炎预后的关系。
方法选取 7 只健康新西兰大白兔,制作标准真菌性角膜炎动物模型。每天在裂隙灯显
微镜下观察兔眼大体改变,分别记录真菌溃疡形成后不同时间段兔眼的病程进展;分别于真
菌溃疡形成后第 3、7、15 和 30 天,行病理学和超微结构检查。
结果兔眼真菌溃疡形成后第 1 天和第 3 天,可见有较重的炎症表现;第 7 天时,均有
大量角膜周边新生血管长入;第 15 天时,球结膜充血减轻,角膜新生血管有所缓解,角膜
溃疡未见减轻;第 30 天时,有较轻的眼部炎症,角膜周边残存少量新生血管。病理学检查
显示:真菌溃疡形成后第 3 天,重度炎症病理表现,毛细血管扩张,无新生血管;第 7 天,
有角膜新生血管形成;第 15 天炎症局限,有成熟的新生血管;第 30 天,新生血管减少。超
微结构观察:真菌溃疡形成后第 3 天和第 7 天,典型炎症的超微结构改变;第 15 天,基质
层仍可见到真菌菌丝,但较前明显减少,角膜上皮细胞核和细胞器异常;第 30 天,轻度炎
症表现,未见真菌菌丝。
结论 1 兔眼真菌性角膜炎在不应用任何抗真菌药物的情况下具有自愈转归的特
点。2 兔眼角膜新生血管与真菌性角膜炎的恢复有相关性。

Study on corneal neovascularization and prognosis
of fungal keratitis

ABSTRACT

Objective To observe the pattern of corneal neovascularization after the rabbit corneal
was infected by fungous. To explore the relationship between the corneal neovascularization
and prognosis of fungal keratitis in rabbit eyes.
Methods We select seven healthy New Zealand white rabbits to make animal models of
fungal keratitis. The changes of rabbit eyes were observed with slit lamp microscope. The
progression of fungal ulcer of rabbit eyes were recorded at different time. Pathological and
ultrastructural examination were taken at 3,7,15,30 days after the formation of fungal ulcers.
Results Heavier inflammation was observed 1 day and 3 days after the formation of
fungal ulcers. There are great deals of revascularizations around the peripheral cornea at 7th
day. The conjunctival congestion was reducing and corneal ulcer was not improved with the
release of corneal neovascularization from the 15th day. At the 30th day, there is less of eye
inflammation and a small amount of peripheral corneal neovascularization. Pathological
