The purpose of this form is to provide a mechanism for a Sub-Intermediary utilized through a Primary Intermediary by a J&pany to provide: (i) relevant information to assist pany in its required due diligence review process and (ii) a Privacy Notice describing how the collected personal information will be processed.

As a prospective Sub-Intermediary to perform services on behalf of a J&pany (the “Company”), you are being asked to provide relevant personal information about your management, key staff, agents and other individuals (collectively referred to as “Individuals”) to pany for due diligence purposes. Where necessary or appropriate, some of this personal information will be confirmed using other information available from pany, affiliates of pany, a designated independent due diligence provider, or publicly available information. You agree to provide this information pliance with applicable data protection laws and certify that you have appropriate permission to provide the information.

Under applicable data protection laws, you and pany are obliged to inform Individuals of the collection and use of their personal information. By signing and submitting this form, you acknowledge and agree to obtain the necessary permissions and to furnish such notice, and agree that pany will not be held responsible for any loss or harm that may arise as a result of your failure to do so.
