山东省药品使用质量管理规范确认工作程序
of rural drinking water sources, protection of drinking water sources in rural areas by the签署意见在10个工作日内报市局，市级局应在10个工作日内进行审核，对符合药品使用质量管理规范现场检查评定标准和本程序的，在市局网站上公示10天，公示期满后未提出异议的，由市级局颁发《符合药品使用质量管理规范确认书》，并予以公告。
（二）经现场检查和审核，不符合药品使用质量管理规范现场检查评定标准和本程序规定的，应当向被检查单位下发《限期整改通知书》，责令被检查单位限期3个月内整改，整改完成后，再次申请现场检查确认。

（一） 已获得《符合药品使用质量管理规范确认书》的药品使用单位变更仓库、药房地址或条件，应向所在地药品监督管理部门（其中三级医院应向市级药品监管部门）提出确认申请，经按照本程序规定进行现场检查后，予以审核确认。
（二）颁发的《符合药品使用质量管理规范确认书》，只确认现场检查的当时符合药品使用质量管理规范，不实行有效期制，两年复检一次。复查时仍然达到标准，继续保留《确认书》，达不到标准，收回《确认书》，责令被检查单位限期整改。
（三）《符合药品使用质量管理规范确认书》由省食品药品监督管理局统一印制。
（四）确认书编号规则
确认书编号格式为：各市简称＋英语字母＋4位年号＋4位流水号，如济A20080001、青B20080002号。
各市简称采用各市名称第一个字，因济南市与济宁市相重，故济宁市简称“宁”。英语字母A表示医疗机构；B表示计划生育技术服务机构；C表示疾病预防控制机构。“2008”表示2008年确认的。4位流水号即顺序号。
