三、制作说明
(一)首部
1、页码。记录完毕，第×页处应逐页填写页码，共×页处应填上总页数。
2、被检查人。被检查人是单位的，应写明单位全称、地址(包括联系地址)、邮政编码、联系电话、法定代表人或负责人姓名、性别、职务等。被检查人是个人的，应写明姓名、性别、年龄、民族、现在工作单位、住址、联系电话等。
3、检查机关。应当填写卫生行政机关名称。
4、检查时间。应写明到现场检查起止时问。即年、月、日、时、分开始至何时何分结束。
5、检查地点。地点不仅包括被检查现场所在地的地址，还包括被检查现场的具体位置。应写清被检查现场所在的地点，如：
