直肠癌前切除术后吻合口瘘的危险因素分析及评分系统建立 中文摘要 II Risk factors of anastomotic leakage after anterior resection of rectal cancer and establishment of a scoring system Abstract Objective: To analyze the risk factors of anastomotic leakage after resection of rectal cancer,explain the pathophysiological mechanisms, and establish a scoring system to guide clinical practice. Methods: The clinical data of 238 rectal cancer patients underwent anterior resection in our hospital from 2012 to 2014 were analyzed. We used univariate analysis and multivariate logistic regression analysis to choose the associated factors, and establishment a scoring system based on clinical data. Results: 19 cases of 238 patients developed anastomotic leakage, the incidence was %. Univariate analysis and multivariate logistic regression analysis showed that age, anastomotic location, lower albumin levels and hemoglobin levels were risk factors of anastomotic leakage after anterior resection of rectal cancer. These patients were devided into high risk, intermediate risk and low risk groups based on scores of 5-7, 3-4 and 0-2. The leakage rates of this three groups were 80%, % and %, respectively (p<). Conclusions: Age, anastomotic location, lower albumin levels and hemoglobin levels are risk factors of anastomotic leakage after anterior resection of rectal cancer. The scoring system is effective for identifying the high risk patients for leakage.
