摘要
目的:本文旨在探讨淋巴结转移个数,转移度及区域数与中晚期胸段食管鳞癌
的预后关系,以及影响淋巴结转移的因素。方法:回顾性分析 2002 年 1 月~2006 年
12 月在新疆肿瘤医院行胸段食管癌根治术 339 例病例。所得结果采用 软件
包分析,计算生存率用 life tables 法,生存率的比较用 Kaplan-Meier 生存曲线和
Log-Rank 检验。淋巴结数量分级、淋巴结转移度用 Classification Tree 分析,影响
淋巴结转移的因素用 Logistic 回归分析。结果:1. 339 例中晚期胸段食管鳞癌的 1、2、
3 年生存率分别为 %,%,%;其中无淋巴结转移的 192 例,有淋巴
结转移的 147 例,两者 1、2、3 年生存率差异有统计学意义(P<)。
数与中晚期胸段食管鳞癌的生存呈负相关(P<);以淋巴结转移数分为 0、1~3、
>3 枚 3 组进行比较,各组 1、2、3 年生存率组间差异有统计学意义(P<)。
结转移度与食管癌的生存呈负相关系( P<)以每例淋巴结转移数 0,~10%,>10%
分 3 组,各组 1、2、3 年生存率比较差异有统计学意义(P<)。4. 淋巴结转移区域
数与中晚期胸段食管鳞癌的生存呈负相关(P<);按淋巴结转移区域数分为 A、
B、C 3 组进行比较各组 1、2、3 年生存率组间差异有统计学意义(P<)。
位、癌的组织分化、浸润深度、脉管瘤栓与淋巴结转移相关。结论:
管鳞癌有淋巴结转移的 1、2、3 年生存率明显低于无淋巴结转移者。
管鳞癌淋巴结转移数量越多、转移度越高或转移区域数越多该病预后越差。
巴结转移的因素为肿瘤部位、浸润深度、癌组织的分化程度及脉管瘤栓。

Abstract
Objective:To study the risk factors of the number of lymph nodes metabasis(LNM),
we investigated the relationship between the number of LNM and prognosis of mid and
late stage squamous-cell carcinoma of thoracic esophageal. Methods:A total of 339
patients were retrospectively analyzed, who were treated by radical correction of thoracic
segments esophageal carcinoma at the Xinjiang Tumor Hospital, Xinjiang, China from Jan.
