拔牙和非拔牙矫治对面部软组织正貌的影响

摘要
目的:本研究的目的在于比较安氏Ⅰ类畸形拔牙与非拔牙矫治前后,面部软组织正
貌相关因素的变化,从而指导正畸治疗的临床方案设计。
方法:收集 60 名 10-18 岁的青少年安氏Ⅰ类错畸形病例正畸前后的正面像、
像,其中拔牙组病例 30 例,非拔牙组病例 30 例。利用 Picasa 3 软件、e- 软件两
组病例正畸前后的正面像、上下颌像测量每一例病人治疗前后牙弓宽度、微笑黑三角、
微笑露齿的数量、放松时口裂相对宽度、放松时上下唇红相对厚度 5 项内容,用配对
t 检验比较出矫治前后的变化,用两独立样本成组 t 检验对比两组之间的差异。
结果:拔牙组矫治后上牙弓前段变宽(P<),上牙弓后段变窄(P<),下牙
弓后段变窄(P<),而下牙弓前段变化无统计学意义(P>)。非拔牙组所引起的
牙弓改变均无统计学意义(P>)。
非拔牙组矫治后颊廓系数变小(P<),因此微笑黑三角变小,而拔牙组的改变
无统计学意义(P>)。
拔牙组和非拔牙组矫治后上颌微笑露齿量均增加(P<),但两组改变之间的差
异无统计学意义(P>)。
拔牙组和非拔牙组矫治后口裂宽度均变大(P<),而两组变化之间的差异无统
计学意义(P>)。
拔牙组矫治后上下唇红相对厚度变小(P<),而非拔牙组的变化无统计学意义
(P>)。
结论:拔牙和非拔牙矫治对软组织正貌存在以上不同的影响,其中拔牙矫治对美观
的综合改善较明显,设计正畸治疗方案时,应综合考虑以上影响。

Abstract
Objective: The purpose of this study was to research the impact of extraction and
nonextraction treatments on the frontal views of the soft tissue in Class I patients, and then to
direct the orthodontists to design orthodontic treatment plans.
Methods: Dental arches and frontal view photographs of 30 extraction and 30
nonextraction orthodontic patients with Class I lusions were measured with the
soft-ware Picasa 3 and e- before and after orthodontic treatment. All the patients are
10~18 years old. There wear 5 factors to be measured on each patients’photographs: the arch
width, the al corridor, the number of exposed teeth when smiling, the width of relaxed
oral fissure, and the correspondence thickness of the upper and lower relaxed lip vermilion.
Paired t-test was used to evaluate the treatment changes within each group. Group t-test of
independed samples was used pare the changes between groups.
Result: In the extraction group, after orthodontic treatment the maxillary anterior arch
width increased (P<) and the maxillary and mandibular posterior arches’width decreased
(P<).
