一、项目名称
药品零售（连锁）企业开办、变更审核
二、设立依据
《药品管理法》及其《药品管理法实施条例》、《药品经营许可证管理办法》、《安徽省药品经营许可证管理办法实施细则》、《关于印发开办药品零售连锁企业验收实施标准及开办药品零售企业验收实施标准的通知》（皖食药监市〔2011〕76号）。
三、申报条件
（一）新办药品零售企业

申办条件
1
零售药店按经营需要设置包括企业负责人、质量管理、采购、验收、养护、保管及营业等岗位，并设置由质量管理、验收及养护人员组成的质量管理机构或专人负责质量管理工作。
职工总数中符合条件的药学专业技术人员不少于2人。

零售药店法定代表人或企业负责人、质量管理负责人无《药品管理法》第76条、83条规定的情形，企业法定代表人或企业负责人、质量管理负责人并经市局考核合格（不参与经营管理人员除外）。

零售药店质量管理负责人必须是注册到本单位的执业药师，并有1年以上（含1年）药品经营质量管理工作经验。
在农村乡镇以下地区设立的零售药店，质量管理负责人应具有(中)药师或药学相关专业初级以上专业技术职称。

经营处方药的零售药店，省辖市及县城所在地的药店处方审核人员必须是注册到本单位的执业(中)药师；县以下地区开办的零售药店的处方审核人员必须具有(中)药师或(中)药师以上专业技术职称，有条件的应配备执业(中)药师。

经营处方药、甲类非处方药的零售药店，从事质量管理工作的人员：县城（含 ）以上应具有(中)药师以上的技术职
