3、所有破损药品必须当日填写破损药品处理单并分析原因。
4、破损药品进行月度汇总。
5、破损药品若为非人为因素，应及时通知医院药品管理及采买人员，当场确认并签字。
6、破损药品若为人为因素，责任人应原价赔赏，以门诊收费单据为准，特别贵重药品可报院长办公会研究解决。
7、所有破损药品应有影像资料及实物，无影像资料的，视为人为破损。
