2015 年，社区居民对家庭责任医师团队提供服务的满意度达到90%以上。
四、工作内容
（一）组建家庭责任医师团队。各社区卫生服务机构按照“分片包干、团队合作、责任到人”的工作机制，根据所辖服务区责任医师团队。家庭责任医师团队以全科医师为主，由全科医师（含中医）、公共卫生医师、社区护士3～5 个人组成，全科医师必须具有执业医师资格，并经过省（市）级全科医学岗位培训合格者担任。
（二）明确服务项目。家庭责任医师团队的主要服务项目包括：基本公共卫生服务、基本医疗服务、家庭健康管理、转介转诊服务等。
1、基本公共卫生服务：完成国家规定的基本公共卫生服务项目，包括建立居民健康档案，健康教育与健康促进，预防接种，传染病防治管理，高血压、糖尿病等慢性病管理，重性精神疾病管理，儿童保健，妇女保健，老年人保健，卫生监督协管等。
2、基本医疗服务：提供一般常见病、多发病的诊疗和护理，诊断明确的慢性病治疗服务；提供家庭出诊、家庭护理、家庭病床等家庭医疗服务；对急性伤病后及手术后的患者、各类残疾人、各种慢性病患者和年老体弱者等提供康复治疗；提供相关的中医药服务。
3、家庭健康管理：对签约家庭的成员提供预防保健服务。为其家庭成员提供养生、运动、营养、健康体检等方案，进行健康评估，制定家庭健康干预计划；提供健康指导、健康咨询、营养咨询、心理咨询、用药咨询等服务。
