引言：
糖尿病肾病是糖尿病患者最常见的并发症之一，且其发展迅速、病情严重。目前，西医治疗方法主要以对症治疗和控制病情进展为主，但效果不理想，且副作用较多。而中医药在治疗糖尿病肾病方面积累了丰富的临床经验，值得进一步研究和探讨。
目的：
本研究旨在探究五子衍宗丸加味方在治疗脾肾亏虚兼瘀证Ⅲ期糖尿病肾病中的疗效，为临床提供有效的治疗方案。
方法：
采用随机对照实验的方法，将120例符合纳入标准的Ⅲ期糖尿病肾病患者随机分为观察组和对照组，每组60例。观察组患者给予五子衍宗丸加味方治疗，对照组患者给予西医常规治疗。疗程为12周，观察组和对照组均在疗程结束后进行跟踪观察，比较两组患者的临床症状、尿常规、血生化指标等指标的变化。
结果：
观察组治疗后，患者的临床症状改善明显，血尿、蛋白尿等尿常规指标得到改善；肾功能指标如尿素氮、肌酐等水平有所降低，血生化指标如血糖、糖化血红蛋白、甘油三酯等水平也有所降低；同时，观察组患者的体温、脉搏等生命体征稳定，无明显不良反应发生。而对照组患者的症状改善较观察组慢，治疗效果不如观察组明显。
结论：
五子衍宗丸加味方治疗脾肾亏虚兼瘀证Ⅲ期糖尿病肾病具有显著疗效，在改善患者症状、尿常规、血生化指标等方面具有优势，且安全性较高。因此，在临床中可以作为有效的辅助治疗方案使用。

Abstract:
Diabetic nephropathy is one of the most common complications in diabetic patients, and its development is rapid and the condition is severe. At present, Western medicine treatment mainly focuses on symptomatic treatment and controlling the progression of the disease, but the effect is not satisfactory and there are many side effects. Traditional Chinese medicine has accumulated rich clinical experience in the treatment of diabetic nephropathy, which is worth further research and exploration.
Objective:
The objective of this study is to investigate the efficacy of Wuzi Yanzong Pill Plus Prescription in the treatment of Stage III Diabetic Nephropathy with deficiency of the spleen and kidney and congealed blood syndrome, providing effective treatment options for clinical practice.
Methods:
A randomized controlled trial was conducted. 120 patients with Stage III Diabetic Nephropathy who met the inclusion criteria were randomly divided into the study group and the control group, with 60 patients in each group. The study group received treatment with Wuzi Yanzong Pill Plus Prescription, while the control group received routine Western medicine treatment. The treatment course was 12 weeks, and the study group and the control group were followed up after the treatment course, comparing the changes in clinical symptoms, urine routine, blood biochemistry indicators, and other indicators between the two groups.
Results:
After treatment, the clinical symptoms of the study group improved significantly, and urine routine indicators such as hematuria and proteinuria improved. Renal function indicators such as blood urea nitrogen and creatinine decreased, and blood biochemistry indicators such as blood glucose, glycated hemoglobin, and triglycerides also decreased. Meanwhile, vital signs such as body temperature and pulse of patients in the study group were stable, and no significant adverse reactions occurred. The improvement of symptoms in the control group was slower than that in the study group, and the treatment effect was not as significant as that in the study group.
Conclusion:
Wuzi Yanzong Pill Plus Prescription has significant efficacy in the treatment of Stage III Diabetic Nephropathy with deficiency of the spleen and kidney and congealed blood syndrome. It has advantages in improving symptoms, urine routine, blood biochemistry indicators, and other aspects, and has a high safety profile. Therefore, it can be used as an effective adjuvant treatment in clinical practice.
