老年颈动脉狭窄与认知功能障碍的相关研究

【摘要】目的通过现状调查观察老年人群中颈动脉狭窄的发生特点及其与认知功能障碍的关系。方法随机选择
2006 年 10 月至 2009 年 2 月,在第三军医大学大坪医院野战外科就诊的 65 岁以上患者 2115 例,经颈动脉超声和数字减影血
管造影( )检查颈动脉狭窄情况,收集临床资料并测试认知功能,行统计学分析。结果颈动脉狭窄好发于颈内动脉颅外
DSA
段(56畅59%)。与无颈动脉狭窄组相比,颈动脉狭窄组高血压病(47畅91% vs畅18畅51%)、糖尿病(38畅26% %)、高尿酸
血症(15畅76% %)、短暂性脑缺血(38畅91% %)、脑白质病变(23畅79% %) 发病率明显较高,差异有统
计学意义(P <0畅01)。随颈动脉狭窄程度增加,患者总分、定向力、即刻回忆力、注意计算力、延迟回忆力和语言能力评
MMSE
分逐渐降低。结论颈动脉狭窄与认知功能障碍明显相关,随颈动脉狭窄程度增加,认知功能损害加重。
