Fixed typographical errors, added hyperlinks, added indexing
Fixed Appendix page numbering to be continuous with the rest of the manual. A side effect of doing this, due to Author-it / MSWord interactions, is that all appendices are “Sections” and numbered in sequence with non-appendix sections.
Fixed changes posted in DocTool. Thanks go to Dana, Cliff, Sam, and Linda.
