第四条 根本医疗保险费缴费基数以上上一年全市城镇非私营单位在岗职工年平均工资〔以下简称上上年市平工资〕为最低缴费基数。
〔一〕单位缴费以用人单位全部在职职工缴费工资总和为缴费基数；在职职工个人按本人上年工资总额作为缴费基数，工资总额低于上上年市平工资的，按上上年市平工资作为缴费基数；超过上上年市平工资300%的局部不计入缴费基数，按上上年市平工资的300%作为缴费基数。
〔二〕个体参保人员以上上年市平工资作为缴费基数。
〔三〕职工个人工资总额的构成以国家统计局规定为准。
第五条 根本医疗保险缴费率为9%。其中：用人单位缴费率为7%，在职职工个人缴费率为2%；个体参保人员缴费率为9%。
第六条 用人单位每年应及时向属地社保经办机构申报应缴纳的单位全部职工缴费工资。未及时按规定申报当年缴费工资的，属地社保经办机构可按上上年市平工资作为在职职工的最低缴费基数。用人单位补办申报手续并经属地社保经办机构核定后，按核定后的工资总额作为缴费基数。
个体参保人员在每年6月30日前向属地社保经办机构申报并足额缴纳当年的根本医疗保险费。
