报名表格填写须知
本报名表格由5部分内容组成:1)报名申请表,2)报名单位联系表,3)报名单位基本情况介绍表,4)主要设计人员履历表,5)报名单位设计人员组成;
报名单位应按照所提供的表格如实填写(表格附后);
联合体报名应按照参与本项目的主次顺序排序,以排在第一位的单位为主体,并须同时提供联合体投标协议书(加盖公章);
报名单位若为分公司,应申报自身的业绩并注明与母公司的合作关系;
报名单位可根据具体内容对表格进行适当调整,但不应改变表格的结构;
报名单位可先将Word格式的报名材料发送至主办方指定电子邮箱,单个附件小于15M,邮箱地址为:104378914@,截止时间为2011年3月22日17:00(北京时间);
报名单位提交的一切报名资料不予退还;
报名单位对报名信息的真实性负责,因提供虚假信息所造成的一切后果一律由报名单位自负。
Notes pleting application forms
1. The application posed by 5 parts: 1) Application for collection, 2) Contact for Collection, 3) Basic Information about Applicant, 4) Introduction of Main Designers Proposed in this Project, 5) Proposed Staff / Assistant Staff Dedicated to this Project;
2. Applicant shall fill out the form provided authenticity (Form attached);
3. monwealth shall participate in the project application the primary and secondary sort order to a unit ranked first as the main body, and shall provide monwealth of Tender Agreement (official seal);
4. If the application for the branch unit, should declare its own performance and indicate the relationship with the pany;
5. Applicants can be adjusted on the table, but should not change the form of the structure;
6. Word format registration unit may first registration materials sent to the host specified e-mail, a single attachment is less than 15M, E-mail: 104378914@, deadline will be 5:00 . on the March 22, 2011 (Beijing time) ;
7. Registration for all applicants to submit non-refundable registration information;
8. Applicants responsible for the authenticity of the registration information, for providing false information for all the consequences arising from the application unit shall be conceited
