三、应提供的审核资料：
1、本人有效身份证、单位提取证明、县（市）区以上具有鉴定资质的房屋安全鉴定部门出具的房屋安全鉴定报告以及乡（镇）人民政府以上职能部门出具的房屋权属证明。
2、单位提取证明提供原件，其他需提供原件及复印件。
3、提取配偶住房公积金的需同时提供配偶的身份证原件及复印件、结婚证原件及复印件、配偶的单位证明原件。
四、提取额度：大修住房的实际发生额。
五、提取方式：转账提取，转账至提取人在缴存银行开立的个人储蓄账户内。
六、提取次数：批准大修住房之日起一年内提取一次。
