Instructions On filling Up The BCBG Factory Profile 2017/2/16 Section Instructions for filling in the Profile and the CT - PAT documents A Please do not fill in the boxes at the top ( BCBG, BCBG Girls, etc) If you are an Agent/pany, please fill in panies name against "Supplier", and contact details against the apropriate fields. B Please fill in the name of the factory / manufacturing unit along with the contact details. Please fill in all the fields. If you use sub - Contractors, then make sure to fill in all their details as well. C Please identify the items that you produce and the fabrics fill in the monthly production capacity against each item that you produce. D Please make sure that you fill in all the fields, especially your minimum quantity requirements by color. E If you are a new factory OR if asa vendor you are proposing a new factory; please do not fill in the boxes against " BCBG Group & Years of doing business". Please fill in the names of your customers in the USA and the years that you have done business with them. Please submit representative samples with indicative FOB / CMT prices and digital photos of the factory showing plete manufacturing process. F Please make sure that you fill in the size of the plant as requested and fill in the number of sewing lines. We would mend that you check these figures before you submit the form to us. G Please fill in the number of managers, supervisors and operators that you have. Please make sure that the numbers mentioned are accurate. H Please give the sizes of all cutting tables. You may attach a machinery list if you need more space than the lines provided and list all the special equiment J Please attach the latest copies of the acctual pliance audit reports and corrective action plans that you currently have on hand that are within the past 12 months.
