临床医学论文-纳洛酮对重型颅脑损伤脑血流动力学及颅内压的影响 【关键词】 纳洛酮 【摘要】 目的 观察纳洛酮对重型颅脑损伤患者脑血流动力学及颅内压的影响。方法 选择重型脑损伤患者84例,随机分为对照组与治疗组。对照组40例,仅采用常规治疗;治疗组44例,采用常规治疗+纳洛酮。治疗过程中应用经颅多普勒(TCD)监测脑血流动力学及应用颅内压监护仪监测颅内压。结果 治疗组脑血流动力学Vs、Vm、PI值在纳洛酮治疗3天后与对照组相比明显改善(P<);治疗组在第3天后平均颅内压明显低于对照组(P<)。结论 大剂量纳洛酮能够改善重症颅脑损伤引起的脑血流动力学紊乱,降低颅内压,改善患者预后。 【关键词】 纳洛酮;颅内压;颅脑损伤;血流动力学 Effect of naloxone on cerebral haemodynamics and intracranial pressure of patients with severe craniocerebral injury 【Abstract】 Objective To investigate naloxone’s effect on cerebral haemodynamics and intracranial pressure(ICP) of patients with severe craniocerebral 80 patients with craniocerebral injury was divided into control group (n=40) and treatment group (n=44).The patient in treatment group were given large dose of naloxone in addition to conventional therapy adopted by those in control group were recevieved conventional haemodynamics was measured with transcranial Doppler(TCD),and intracranial pressure(ICP) was monitored by monitor intracranial pressure Vs,Vm and PI in cerebral haemodynamics in treatment group were improved obviously (P<) after 3 day treatment,and ICP in treatment group was lower after 3 day treatment (P<).Conclusion Large dose of naloxone can improve cerebral haemodynamics,decrease ICP,and improve the prognosis of severe craniocerebral i
