经胸腔镜心包开窗治疗恶性心包积液31例疗效分析

[摘要] 目的对经胸腔镜心包开窗治疗恶性心包积液的治疗效果予以探讨。方法回顾分析自2007年9月―2016年6月在该院住院接受经胸腔镜心包开窗手术治疗的各种恶性心包积液患者共31例,对其临床资料开展回顾分析。结果全组31例患者无围手术期死亡及严重持久性并发症发生,术后胸腔引流2~8 d, d,全组无拔管困难,术后随访均超过3个月,无心包积液复发。结论经胸腔镜行心包开窗治疗恶性心包积液是一种安全有效的治疗手段,值得在临床应用中推广。

[Abstract] Objective To study the effect of video-assisted pericardial window in treatment of malignant pericardial effusion. Methods 31 cases of patients with malignant pericardial effusion for video-assisted pericardial window in our department from September 2007 to June 2016 were retrospectively analyzed and the clinical data were retrospectively analyzed. Results There were no death patients and patients with severe and plications during the perioperative period, and the chest drainage was 2 d to 8 d after operation and the average time was d, and there was no difficulty in decannulation in the whole group, and the patients were followed up more than 3 months, and no pericardial effusion recurred. Conclusion The video-assisted pericardial window in treatment of malignant pericardial effusion is a safe and effective treatment means, which is worth promotion in clinical application.

心包积液是一种常见临床病症,常见病因有非特异性、结核性、化脓性和风湿性、病毒性以及低蛋白血症、心衰、肿瘤、外伤等,而由心包的原发恶性肿瘤以及其他部位的?盒灾琢鲎?移至心包或直接侵犯心包而引起的心包腔液体过度积聚,称为恶性心包积液[1-2],恶性心包积液一般增长迅速,量也比较大,常可达一两千毫升甚至更多,会对患者的生理及心理健康产生严重的负面影响,使患者的生活质量急剧下降,如果不能有效解除心包积液引起的心脏压塞,患者很快会因为循环衰竭而死亡,该院自2007年9月―2016年6月经胸腔镜行心包开窗治疗恶性心包积液31例,取得较好疗效,现报道如下。
1 资料与方法
